It's Federal Law: Use of the VISs

As required under the National Childhood Vaccine Injury Act, "all health care providers in the United States who administer any vaccine containing diphtheria, tetanus, pertussis, Haemophilus influenzae type b (Hib), varicella, measles, mumps, rubella, polio, or hepatitis B vaccine shall, prior to administration of each dose of the vaccine, provide a copy of the relevant vaccine information materials (known as VISs) that have been produced by the Centers for Disease Control and Prevention to the patient or parent/guardian."
