ARNP Global Signature Bill
With HB 1259, sponsored by Representative Cody, ARNPs United is moving forward to help enable all ARNPs to sign a variety of forms and documents in Washington State which have previously required the signature of a physician. The goal is to require that the signature of an ARNP be accepted on any non-federal form that is within the scope of practice for the signing ARNP.

Stop the Silence: The Impact of Clinicians and Intimate Partner Violence

Background
Intimate partner violence (IPV) is a major public health problem across the country with alarming statistic affecting individuals, families and communities. In the United States, 24 persons per minute are victims of sexual assault, physical violence, or stalking by an intimate partner. In 2011, there were 47,444 domestic violence offenses reported in Washington State (Department of Health, 2013). According to the Washington Department of Health approximately 70-80 percent of victims are female, and one out of four females will be a victim of some type of abuse in her lifetime (2013). The national cost for IPV is over $8 billion annually with most funds going to direct and indirect healthcare costs e.g. medical treatment for physical injuries, psychological treatment and loss of productivity from work (CDC, 2014). Identifying current or past abusive and traumatic experiences can help prevent further abuse, lessen disability, and lead to improved health status (Office on Women’s Health, 2013).

Health Effects of IPV

70% more likely  to have heart disease
80% more likely to have a stroke
Twice as likely to be a current smoker
Twice as likely to suffer from depression and headaches
Twice as likely to have alcohol use disorders
16% more likely to have a low birth-weight baby
42% of women who have experienced physical or sexual violence at the hands of a partner have experienced injuries as a result
38% of all murders globally were reported as being committed by their intimate partner

Current Screening Recommendations
One of the preventive services identified by the Institute of Medicine is screening and counseling for “interpersonal and domestic violence.” Under the Affordable Care Act, these services are generally covered in new health plans without requiring a co-payment, co-insurance, or deductible (Health Resources and Services Administration, 2012; The Patient Protection and Affordable Care Act, § 2713).  Additionally, the U.S. Preventive Services Task Force (USPSTF) released a recommendation in January 2013 calling for clinicians to “screen women of childbearing age for intimate partner violence” (U.S. Preventive Services Task Force, 2013).

What Can Health Care Providers Do?
Because they are often trusted resources in their communities, health care providers are in a unique position to connect women who experience interpersonal and domestic violence with support (OWH, 2013).   Clinicians usually see patients individually, giving patients the ability to talk to someone without the abuser present (ASPE, 2013).  The health care environment and interaction with a provider may be a victim’s only contact with the outside world which emphasizes the importance of screening.

Just as providers routinely screen patients for diabetes or high blood pressure and refer them to specialists as needed, providers can also screen for interpersonal and domestic violence and provide a referral to local domestic violence programs and services. If a woman discloses abuse, the provider can provide brief counseling to 1) promote the patient’s immediate safety; 2) discuss the possible relationship between current or previous interpersonal and domestic violence and the patient’s health concerns; and 3) link the patient to support services and resources (OWH, 2013). Women who talk to their provider about abuse are 4x more likely to seek help. Ask your patients about abuse and violence (National Health Collaboration of Violence and Abuse).
