HealthReach Community Health Centers offers four unique opportunities for
FNP’s to live and practice in rural Maine communities as follows:

ALBION,  MAINE – Lovejoy Health Center
Lovejoy is one of the oldest outpatient practices in the HealthReach system,
providing the full scope of Family Practice, including obstetrics.

STRONG, MAINE – Strong Are Health Center
Assist two providers in this outpatient practice.  Provide healthcare and
refer patients for dental care within the same facility which serves over
2000 people annually.

LIVERMORE FALLS, MAINE – Western Maine Family Health Center
Join five providers in providing medical services, which assure that both
individuals and family members receive the convenient and comprehensive
attention that they deserve.  Health center patients requiring admission to
Franklin Memorial Hospital receive care from a FMH hospitalist who
communicates with the patient’s regular provider at the health center during
hospitalization and upon discharge.

FLOAT FAMILY NURSE PRACTITIONER – HealthReach Community Health Centers
Fill in as needed at eleven community health centers in central and western
Maine providing primary care and preventive services to patients of all
ages.  The selected candidate will be an independent FNP with excellent
clinical skills.  This is a permanent benefited position with a set number
of hours (TBD) and a generous differential.
