Here is a short section from my book, Nurse Practitioner Certification and 
Practice Preparation, 2d ed., on the B12 dose issue.

The usual initial B12 dose is 100 mcg/d intramuscularly for the first week, 
then weekly for the first month, and then monthly for the rest of the 
patient's life.  Traditionally, doses as high as 1000 ug per injection have 
been used. However, a cyanocobalamin dose of more than 100 ug in a single 
injection exceeds the TC II's binding capacity; excess is excreted via the 
kidney and wasted.
