I too would have referred her back to her PCP for further evaluation.? She may very well not have Anorexia, however, you can't determine she doesn't without documentation of previous height and weights and a more thorough evaluation.

I couldn't find any guidelines while doing a search, but I did run across this article which may be helpful, plus, it has CME.

I would deny athletic clearance based on her BMI. This is VERY much
underweight, and she needs more extensive evaluation than what you could
provide at a "mass school physical" event.

Subject: [NP-Clinical] Sports PE question

I wondered if anyone has handy the guidelines on deferring athletic
clearance on BMI. I had a student who I strongly suspect has AN. Of
course, she denies and has always been thin. Was in serious MVA last year
and was cleared by neuro but has gone from BMI of 17.2 to 14.9. Breath was
on the ketotic side, heart exam normal but really skeletal appearing. She
denied any health problems but I deferred her clearance to her PCP. This
was at our high school so I'm not her PCP.
I couldn't find any standards and Lorraine's book is on loan so not handy.
It has a great sports PE and went to her NPACE workshop which was great but
we didn't touch on BMI.
Thanks for any advice. I hated to be the heavy and don't know the family
but followed my gut.
