Hi,
Even though her thrombi have been post surgical, has she been tested or aware if she has a congenital protein C or S deficiency? Either way, hematology referral sounds like the way to go. Carla Anderson/Portland OR

I need some help trying to figure out whether my patient needs to be
anticoagulated for life. This is the 3rd DVT that patient has had since
2000--all have been post-surgical. Pt had IVC filter placed after 2nd
episode. By patient history (she is also a nurse), she did not receive any
heparin or Lovenox after her surgery. She also has a hx of RA, currently on
a DMARD (can't remember which one) and Prednisone 5 mg QD. My feeling is
that with her hx of multiple DVTs and the fact that she also has an
inflammatory disease, she likely needs lifelong anticoagulation. However,
the fact that they have all been post-surgical is making me question that.
I will very likely refer to hematology to get a recommendation to cover my
rear end, since anticoagulation is fraught with so many potential
complications. But I appreciate any insight before that...
TIA,
Cherie Wright, FNP

Angie--
You have to go to your survey, click on the brown box next to it to open it
up for participants. Otherwise, it's going to give everyone the message that
the site is invalid or closed and you aren't going to get anywhere!

I am sorry that there was a problem with my first
email. Here is another try at the attachment for my
research study on NP's and national asthma guidelines.
I very much appreciate your participation.
Again, the survey will take about 20 minutes and is
for those who work with children in primary care.
Thanks,
Angie
