  I am very sorry about your friend who was in remission, that now has recurrence.  Oncology is not my field at all, but I am going through lung cancer with my dad, and my mom is getting a biopsy next week for a suspicious breast mass next week. (my mom does not live in this state)  The news before Christmas for your friend sounds like it was awful.  I do know someone who works full time, and has a similar situation with mets to her spine, but she has done well for a couple years, I believe with palliative chemo.  Thats a positive thing, wish I knew more.  Carla Anderson/Portland,OR

Oncology is not my area of expertise either. However, I had been a hospice 
nurse for 5 years. In a nutshell, primary sites found early and treated may 
be cured. Metastatic sites typically are much more ominous. I am unaware 
of any new treatment that changes that rationale. However, I have been out 
of the loop for awhile.

I agree, timing was poor. I carefully consider when I make phone calls. 
Never on a Friday and particularly not before a holiday.

I must say, that I have had recent experience with a patient that was 
nothing short of miraculous. 44 year old with presumably inoperable lung 
mass and given 6 month life expectancy. Spontaneously regressed well after 
chemo had been completed. Left lung removed and all nodes negative. My 
best Christmas present was a handmade bracelet from her. It will always 
signify the possibilities of a miracle to me.

Agree with Dena that Afrin can be key. Just make sure to educate the pt not
to use for longer than 3 days due to risk of rebound congestion. I "cheated"
one time with a bad cold/sinus infection and used for 7 days and had some of
the worst congestion of my life when I tried to d/c it!

Any miracle treatment for these pts with chronic plugged ear - Eustachian
tube dysfunction, after a URI? I see several of these a week and always just
advise Guafenison 400- 600 BID and Sudafed. Any other ideas? I have one lady
that is driving me nuts and the TM looks perfect - just this clogged feeling
with some popping.

I agree with advice regarding Sudafed and Afrin. This is what I tell my pts. to try. Studies (at least regarding Sudafed--don't know if there have been any with Afrin) have not shown any benefit. I tell my pt's that the meds are worth a try, but that time will resolve their symptoms and that it can take a month. I do tell pts. that if their symptoms have not resolved in a month, to seek ENT eval. The fullness and popping are usually more of an annoyance than disruptive, but people have a hard time waiting it out anyway...
