I've used Zyrtec before, too. However, most of my pt population is
uninsured, so generic Benadryl is usually more affordable for them. Good
point to bring out, though, as many will prefer the once daily dosing of
Zyrtec.

>Diane,
Instead of the Benadryl, I use Zyrtec so the dosing is easier and zyrtec is
indicated for hives. The rest of the regimen I use is the same as you.

>For hives, I usually do Benadryl 50 mgQ6hrs around the clock for 2 days
(and then Q6 prn, for H1 blocking),famotidine 20 mg or ranitidine 150 mg BID
(for H2 blocking), and then add in aMedrol dose pack for mild cases or
prednisone taper for more severe cases. Justmake sure she’s drinking plenty
of fluids and that she’s usinghumidifier (and changing filter/cleaning
frequently to help prevent mold) atnight to help counteract the drying
effects of the antihistamines, especiallygiven her sinus infection and the
need to promote drainage.

antibiotic for four days, put on azythromycin for 10 daysyesterday (5 days
on and wait 5 days, then do the next pack for fivedays), and Alavert
(loratadine 10mg qd), still hives but not as much. Whatnext? Continue with
