The prevalence of MGUS in people over 70 yrs is 5.3% - so not really too
uncommon.
Progression to MM or a related malignancy is one percent per year - so
when he's 90 his risk of progression will make it to 20%.

20% progress to myeloma. Depends on the definition of "modest". But 80%
don't
progress, so odds are in his favor.
