[NP-Clinical] Adrenal hyperplasia

Anyone with experience treating patients with adrenal hyperplasia?  No specific mass noted on CT, but adrenal hyperplasia (R greater than L).  Second CT 11months later shows still present and no specific mass.  Patient on Toprol XL and Clonidine (?) for BP.  Do you refer to endo or treat in Internal Medicine or watch.  The patient's (a 60 yr old BM) nephrologist suggested treating or using spironolactone before, but suggested that the Internal Med MD do it.  The Internal Med doc never started the med. Any ideas,
