Part-time Psychiatric Nurse Practitioner

Assess patients referred for admission to the ACT Team & determine 
their functioning status
& service needs. Including: providing written report based on the 
initial assessment,
developing treatment plan, participating in home visits & emergency 
consultation services,
monitoring treatment, reviewing lab & other medical reports, 
prescribing & monitoring meds
and participating in OMH approved ACT Orientations & Training's. 
Queens and Bronx
locations. 14 hours per week.

Must be licensed and registered in NYS. Some experience working with 
people with severe
and persistent mental illness is preferred. Valid driver's license 
and access to a car is helpful.
