As a Nurse Practitioner, you have a wealth of knowledge and

We are seeking a field-based Nurse Practitioner to join us in our 
Staten Island, NY team. In
this fulfilling role, you would visit long-term-care facilities to 
provide the best care possible to
our aging and chronic-condition members, while communicating with 
patients' families and
physicians. This is a challenging role that requires a caring 
professional with a desire to make
a positive impact within the industry.

* Primary responsibilities include:
* Visiting long-term care patients
* Working collaboratively with physicians
* Enhancing educational programs
* Building relationships and maintaining positive communication

The chosen candidate must be a Nurse Practitioner with a current, 
unrestricted license. A
national certification in family, adult, or geriatric practice is 
highly preferred (although other
specialties will be considered). Long-term-care/nursing home 
experience will be very helpful,
and prior geriatric or family care experience is a plus. A Master's 
degree is required.
