Our Nurse Practitioners provide Primary Care within the post- acute / 
skilled nursing centers
that we serve. In this role, you'll provide on-going medical 
management for acute / episodic
rehabilitation patients. Working as a member of the interdisciplinary 
team, you'll meet the
medical needs of patients by developing, updating and evaluating 
treatment plans,
medications and other necessary care.

At least one year in Nurse Practitioner role, prior nursing home or 
geriatric experience
preferred.
