We are looking for several self-directed Nurse Practitioners to 
provide primary care services
to elderly residents in long-term care facilities. Supported by 
physicians, peer NPs and a
clinical advisor, you would enjoy significant autonomy, an ideal 
working environment with

Primary responsibilities include:

* Managing chronic and stable conditions of patients
* Working collaboratively with physicians to develop, discuss, and 
monitor care plan
* Participating in voluntary on-call rotation
* Interacting regularly with the care staff at the facilities
* Documenting all activity
* Attending rounds with physicians
* Participating in care conferences with patients and their families
* Attending monthly meetings

This role requires:

* National NP certification in Geriatric, Adult, or Family care 
(Geriatric certification highly
preferred)
* Excellent communication skills
* Computer proficiency

Highly valued assets include:

* Acute care and direct care experience in a long-term care setting
* Familiarity with Medicare regulations

***Recent NP graduates with RN experience in a long-term care 
setting, skilled nursing
facility, and/or with a geriatric population will also be considered. ***
