NP’s and CNS’s are invited to join a growing, 
innovative, community based geriatric care
program in the scenic Chippewa Valley area. 
Designed to keep frail elderly and adult
physically disabled individuals in their own 
homes as long as possible, this case
management type position is integral in providing 
primary care services within an
interdisciplinary team setting. You’ll enroll 
members (performing initial Hx and PE) and visit
them in their homes periodically, as well as work 
in conjunction with primary MD and team
members to manage chronic diseases and provide 
education. Candidates should hold
family, adult, or geriatric NP or CNS, and valid 
driver’s license. New grads with minimum 2
years nursing experience welcome.
