managed care program in
the scenic Chippewa Valley, west central area of WI. Designed to keep 
frail elderly or disabled
adult pts in their own homes as long as possible, this case 
management type position needs
a computer literate, organized, team player to assist with enrolling 
pts (performing initial Hx
and PE) and visiting the pts in their homes periodically. NP will 
work in conjunction with
primary MD, and the other team members to manage chronic diseases and 
prevent acute
exacerbations of such. Candidates should hold family, adult, or 
geriatric NP or CNS. New
grads are always welcome and great mentors are provided! Salary range 
is $70-$75K range,
excellent benefit/health package, and relocation. Prescriber 
authority within a year of hire date
