(DeFuniak Springs, Chipley, Marianna, Homestead and Hialeah). We are 
actively recruiting for
a nationally certified Nurse Practitioner who will be located 
in-house at one our facilities M-F.

The Nurse Practitioner/Assistant Medical Director will be involved in 
resident assessment for
initial admissions, monitor for changes and assist in assessment 
prior to discharge etc....
This position will involve about 75% direct care with residents and 
approximately 25% of time
in meetings, documenting or developing and mentoring Clinical staff, 
as well as obtain a
Medicare Provider number. This position reports to the offsite 
Medical Director (Consulting
Physician) of the facility and the onsite Administrator of the 
facility and is a partner to the
Director of Nursing (DON) in the Building.
