General Purpose: Under the general direction of the interventional
endoscopists, and with
latitude for exercising independent judgment and discretion,
coordinate and facilitate clinical
and endoscopic care to patients with pancreaticobiliary diseases.
The Yale interventional endoscopy program is one of the busiest
tertiary programs in New
England. Patients with complex pancreaticobiliary problems are
referred by other
gastroenterologists for consultations as well as for procedures.
Referral to the practice is
often the entry point into the YNHH system.
Position Description: Coordinate patient referrals: make sure that
patients referred to YNHH
have in their possession appropriate records, CT scans and labs.
Review and adjust
medications that may interfere with the performance of procedures
such as anticoagulants
and insulin. If necessary, confer with patient's primary care doctor
or specialist to ensure
appropriate medications are given or withheld. Facilitate transfer of
patients from other
hospitals. Assist with communication between the referring hospital
and physician, the YNHH
admitting office, diagnostic imaging and the GI procedure center so
that GI staffing and
scheduling can be arranged for prompt performance of procedures.
Assist with patient related
issues on the hospital floor. While physicians are tied up in the GI
lab or X-ray department all
day, meet with patient families, review medications, evaluate any
changes and report to the
physicians. Screen consultations. Under supervision of the
interventional endoscopist,
evaluate new patients and book urgent cases in a timely fashion for
the same day. Encourage
expeditious performance of procedures, prompt discharge and
cost-effective care. Coordinate
anesthesia scheduling. Facilitate this process, thereby freeing up
physicians to see patients
and facilitate prompt procedure performance. Arrange patient
discharges, ensure appropriate
directions, prescriptions, follow up appointments and procedures are
scheduled. Manage
calls from referring physicians and patients. Promptly return calls
from referring physicians
and their offices, as well as patients, while the interventional
endoscopists are occupied
doing procedures. Educate patients and families about
pancreaticobiliary procedures, their
complexity and the significant risk of complications. Make nurses on
the floor aware of the
broad ramifications of these procedures and able to recognize and
react appropriately to
complications that may arise. Take an important teaching role and
relay this information to
the house staff physicians and nursing staff. Coordinate a physician
conferences: a number
of of CME conferences regarding pancreaticobiliary endoscopy,
including ERCP workshops
and the Digestive Disease Week Review are hosted by the program
yearly. Assist with the
logistics involved in hosting these conferences. Coordinate
appropriate imaging tests, labs
and consultation services. In conjunction with the interventional
endoscopist, schedule and
follow up on appropriate tests and initiate consultations to other
specialties here at YNHH
(oncology, radiology, or surgery). Relay this information to the
referring physicians in a timely
fashion.
Qualifications: Master's Degree in Nursing. Successful completion of
a recognized Nurse
Practitioner program. Licensure in CT as APRN.
