Family Nurse Practitioners

Job Description
The Take Care Nurse Practitioner (TCNP) believes 
in excellence and is passionate about
providing outstanding patient care. The TCNP will 
be responsible for providing episodic care,
screening, and education for patients 18 months 
and older. Our professional clinics are
located at select Walgreens, are easily 
accessible to our patients, and opened 363 days a
year (closed for Christmas and Thanksgiving). 
TCNPs are the heart and soul of our Nurse
Practitioner-based company; a health care model 
committed to promoting patient-centered
communication and care.

Our Take Care Centers are conveniently located 
inside Walgreens retail pharmacies
throughout the Indianapolis, IN area.

Essential Functions
* Provide episodic care, screening, and 
educational support to Take Care Health Clinic
patients.
* Provide diagnostic screenings, such as 
cholesterol and blood glucose, as outlined in the
TCHS evidence-based guidelines.
* Provide common vaccinations such as TB, 
Hepatitis B, Meningitis, and Influenza as
outlined in the TCHS evidence-based guidelines.
* Support and follow the most current 
evidence-based guidelines of practice to provide
quality patient care.
* Fully embrace and utilize TCHS’ latest 
technology to provide patients with access to their
medical records and timely, accurate prescriptions.
* Participate in ongoing quality assurance Peer Review audits.
* Extend warm, compassionate care to all with a respect for patient diversity.
* Maintain commitment to excellence, best 
practices, and superior customer service with
every patient.
* Maintain current state Nurse Practitioner licensure.
* Participate in ongoing clinical education.
* Keep up-to-date on Nurse Practitioner public policy.
* Participate in a collegial fashion with all Take Care Health colleagues.
* Continuously communicate with TCHS leadership 
to enhance patient-centric experiences
at all Take Care Health Clinics.
* Complete training on TCHS’ electronic medical 
record (EMR) and information systems;
utilize the systems to the fullest capacity to 
provide superior patient care documentation.
* Perform other duties as deemed necessary by the 
Assistant Lead NP, Lead NP, Regional
NP, and Chief Nurse Practitioner Officer.
