ADVANCED PRACTICE NURSES
We are seeking nurses for these rapidly expanding 
programs in northern Cook and Lake
Counties:

Medical Home Visits - Provide care to homebound 
patients by assessing medical status
and providing follow up care, working in conjunction with a primary physician.

Palliative Care Consultation - Join a team of 
palliative care specialists to provide consultation
and care to patients living with life-threatening 
or serious and complex illnesses.

Hospice, home health, oncology, primary care or 
palliative care experience preferred.
