A fulltime permanent position is available in MI along the shoreline 
of Lake Saint Clair. A FNP,
ANP, or GNP is needed to work with the geriatric population providing 
preventative care. There
are 80-150 pts on a provider's panel. The goal is to keep residents 
of skilled nursing facilities
and assisted living centers healthy by early identification and 
management of illness. Utilize a
collaborating MD, but may also work with the patient's primary care 
MD if they would like to
retain them. See six to seven patients per day driving to one to four 
facilities. A computer is
provided for documentation, and travel is compensated. New graduates 
with interest in case
management are welcome to apply. Salary ranges from 90- 100K 
depending on experience
with great benefits offered.
