$5,000 Sign-on Bonus! Family Nurse Practitioner (Wellness/Sports 
Medicine/Family Practice)

We are seeking an experienced Family Nurse Practitioner with a Health 
and Wellness /Sports
Medicine background to join a predominantly family practice 
environment with an emphasis in
Wellness and Sports Medicine. The clinic hours are Mon-Fri 
(8:30am-5:00pm) with an option of
potentially working 4X10 hour shifts. The estimated patient volume 
would be 25 patients per day
with on-site physician supervision. The provider will be expected to 
manage patients
independently with minimal supervision. Generous compensation package 
including a base
salary of $95-100K+ (D.O.E.) with additional incentives of 10-15K 
based on productivity for a total
package of approximately $115K. Full Medical, Dental, 401K with 
match. $5,000 Sign-on bonus
and relocation package available for the qualified candidate. Great
