$5,000 Sign-on Bonus! Retail Clinic - Family 
Nurse Practitioner – Eastern Kentucky
(Experienced Providers Only)

We are seeking a Family Nurse Practitioner for a 
retail clinic in Eastern Kentucky. Standard
Clinic hours are Mon-Thurs (9:00am-7:00pm) and 
every other Saturday and Sunday with no call
responsibilities. There is limited supervision so 
the provider must be able to manage patients
autonomously. The patient mix would be a 
traditional family practice setting (with the exception
of pediatric patients below 18 months) including 
chronic diseases Diabetes, Hypertension, etc,
minor illnesses including upper respiratory 
infections, colds, flu etc. as well as acute care.
There is EMR (training provided) and physician 
support via back-up call. The estimated patient
volume would be 10-15 per shift. Base salary from 
78K-95K+ (D.O.E), full benefits package
including Medical, Dental, 403b with a 4% match. 
Relocation package and $5,000 Sign-on
bonus available for the right candidate. Great
