Gila River Health Care is a rapidly expanding 
integrated Health System that is uniquely position
as a 501-C3 tribal entity, with revenues of 100 
million whose mission is to serve a large Native
American population. Services include a CAH, 
ambulatory system with broad range of services;
expansion plans for a 3rd campus in 2011 that 
will establish the organization as a key provider.
We are seeking the following professional:

Nurse Practitioner - Diabetes Care Program

As primary health care provider assesses the 
physical needs of the patient and implement a
plan of care through diagnostic, preventative and 
therapeutic services responsive to the diverse
needs of families and the community in accordance 
with all Tribal, state, federal and licensing
standards and guidelines.

Minimum Qualifications:
* Master’s degree in Nursing preferably with an 
emphasis as a Family Nurse Practitioner,
Pediatric Nurse Practitioner or Public Health Nurse Practitioner
* Current State of Arizona RN license; American 
Heart Association BLS and ACLS certification
within 30 days of hire
* Current license to practice as a Family Nurse 
Practitioner and state authority for advanced
practice including possession of prescription and dispensing privileges
* Must have at least 3-5 years previous Public 
Health, Community Health or primary health care
experience; current and valid State of Arizona driver’s license

Essential Functions and Responsibilities:
* Follows DCP pathways and algorithms or other 
appropriate therapeutic guidelines to help
achieve the recommended goals of the DCP core indicators
* Performs various diagnostic, preventive and/or 
therapeutic procedures and/or orders special
screenings and developmental or laboratory tests 
to diagnose conditions; interprets results
and takes appropriate action
* Serves as a direct link between the family, 
medical providers, community service agencies
and other applicable services/resources to 
provide for health needs of patients
* Secures health and developmental history from 
patient or parent; records findings,
assessment/evaluation of condition to 
discriminate between normal and abnormal findings
* Performs emergency services and/or crisis 
intervention in absence of physician up to and
including life saving procedures to ensure 
stabilization of patient for transport
* Provides age appropriate care and services for 
emergent, urgent, acute or chronically ill or
injured patients; adjusts treatment within 
established standing orders; refers patient to other
health care providers as appropriate
* Works collaboratively and cooperatively with 
other professionals and agencies to coordinate
care and services to include immunizations and 
disease contact, environmental health and
health education
* Works collaboratively with physicians in 
management of complex medical problems
* Participates in planning, instituting, 
evaluating and revising medical program plans; assists
determine conditions, resources and policies for 
efficient delivery of health services
* Participates in and conducts studies relevant 
to health care needs of patients
* Serves in a teaching capacity to facilitate the 
ongoing development of other health care workers
* Maintains records and reports on patients, 
clinic activities and status associated with the clinic
operations, quality assurance and outcome measures
* Works closely and cooperatively with other 
health care providers and outside agencies
including, but not limited to: Child Protective 
Services, RHBA, Tribal Social Services and other
areas as needed to ensure timely communication of 
issues/concerns and coordination of care
* Actively participates in the planning, 
coordination, implementation and evaluation of annual
health fairs, vision/hearing checks, immunization 
clinics and wellness screening programs
* Maintains knowledge and proficiency in medical 
practices through continuing education, staff
meetings and workshop participation
* Implements and evaluates immunization program 
status to ensure compliance with all
regulations and requirements
* Maintains close contact with patient through 
home, clinic office visits, phone and/or written to
monitor progress and suitability of environment
* Participates in departmental orientation, on 
the job training and quality assurance programs/
initiatives
* Participates in a variety of department and 
hospital educational programs to maintain current
skills and competency levels; identifies and 
discusses performance or training needs with
Supervisor
