Nurse Practitioner or Physician Assistant
Heart Failure Clinic

The CardioVascular Institute of North Colorado has an opportunity for 
a Nurse Practitioner or
Physician Assistant in our Heart Failure Clinic. The CardioVascular 
Institute is located within
North Colorado Medical Center, a Magnet Facility, in Greeley, CO. 
Greeley is a beautiful
community of approximately 90,000 with a home town atmosphere. We 
have a desirable climate
and are located along the Front Range of the Colorado Rockies with 
easy access to a wide
variety of outdoor activities, approximately one hour from Denver. We 
are a non-profit healthcare
organization and our professionals are nationally recognized for 
excellence in myriad programs.

The Heart Failure Clinic is a disease management program caring for 
patients with advanced
heart disease. Using a multi- disciplinary model, the clinic closely 
follows about 350 patients with
heart failure. This mid-level provider will be responsible for 
providing patient assessment through
patient exams, patient medical history, interpretation of required 
diagnostic procedures, and
developing/implementing treatment plans with the assistance of 
Clinical Pharmacists, Registered
Dietitians, Cardiologists and primary care providers. In addition to 
the primary emphasis on
outpatient management, there will be inpatient and staff development 
opportunities as well.

Minimum Qualifications:
* Nurse Practitioner or Physician Assistant licensure

Preferred Qualifications:
* One year cardiology experience
* Basic EKG interpretation skills
* May accept a new grad with some experience
* Must possess or be eligible to obtain Medicare UPIN, NPI and DEA 
credentials in his or her
own name
* ACLS certification

Please note: Nurse Practitioners and Physician Assistants providing 
services must be approved
by the Credentials Committee and be granted hospital privileges.
