Position Details:
* Identify, assess and coordinate the care of select Hematological 
Malignancy and Blood and
Marrow Transplant adult patient population comprehensive 
family-centered discharge planning
in an efficient and cost effective manner.
* Identify patient with complex healthcare/rehab needs.
* Assess individual patient and patient populations to identify 
care/education/rehab needs and
develops, implements and evaluates programs to meet these needs and 
coordinate patient
care/collaborates with others.
* Practices utilization management.
* Patient documentation.
* Actively participates in the quality improvement process.
* Involved in learning regarding trends in case management.
* Data analysis.

* Initiates and maintains positive relationships with patients/customers.
* Initiates and maintains positive relationships with co-workers.
* Takes responsibility for self-development and supports a learning 
environment.
* Displays commitment to the mission of the hospital and its values.

Position Requirements:
* Demonstrated competence in identified area of expertise.
* Demonstrates strong interpersonal and leadership skills.
* Demonstrates a professional demeanor when responding to patients.
* Uses a wide variety of technical equipment to meet education and 
patient care needs
(eg. Stethoscope, cardio-respiratory monitor).

Educational Requirements:
Nursing Doctorate or Master of Science in Nursing with Advanced 
Practice Certification

License Requirements:
* Registered Nurse with current license to practice in the State of Ohio;
* Certified as a Nurse Practitioner by a certifying body approved by 
the Ohio Board of Nursing;
* Valid and active Certificate of Authority to practice in the State 
of Ohio by the Ohio Board of
Nursing;
* Valid and active Certificate to Prescribe in the State of Ohio by 
the Ohio Board Nursing (when
applicable)
