Sound Physicians partners seamlessly with 
hospitals, long term acute care & skilled nursing
facilities to provide excellence in clinical 
processes. Transitions in care – supported by
knowledge NP’s – are critical to the services we 
provide to 30 + skilled nursing facilities
throughout the greater Phoenix area.

We have an excellent opportunity for NP to 
provide on-call coverage exclusively via telephone
for skilled nursing facilities (SNF) serviced by 
Sound Physicians’ post acute physicians.

Your Position
* Steady work w/ shifts scheduled 8 PM – 8 AM – 
position is for up to 7 nights per week
* Under the direction of 1 of Sound Physicians’ 
Post Acute Physician & through the use of
approved protocols, you will be responsible for 
triaging the health care needs of the designated
SNF’s patients & documenting the appropriate disposition of each case
* You will be an essential component in the 
provision of care for patients from your & will be
expected to use your knowledge & skills in 
providing medical consultation to SNF nursing
personnel, triage protocols and guidelines
* The demonstrated knowledge to observe and 
assist in the assessment of patients; detect &
report obvious and/or dangerous conditions of 
patients, excellent verbal and interpersonal
communication skills, professional manner/calm 
demeanor, ability to work independently +
multi-tasking abilities will distinguish the successful candidate

support of post acute physicians as our

quality in the post-acute care environment.

JOB DESCRIPTION
Requirements
The successful Nurse Practitioner candidate will 
have a certificate of completion from an
approved program for Nurse Practitioners and be 
eligible for or certified by the Arizona State
Board of Nursing. Previous nurse practitioner in 
a skilled nursing facility or with geriatric
patients is preferred. Ability to work 
independently being continually available by e-mail,
beeper, and/or telephone for the designated work period.

Requires the ability to be alert late hours and 
be tolerant of occasional high volume of calls.

Qualifications
* Excellent verbal and interpersonal communication skills
* Demonstrates a concern for the health and well-being of others
* Must remain calm in stressful situations in 
order to carry out duties in a professional manner
* Superior written and oral communication skills
* Must have current BLS
