Pediatric Nurse Practitioner
Care Coordinator/Patient Care

The Department of Pediatrics at Park Nicollet 
Clinic – Plymouth is seeking a dedicated Pediatric
Nurse Practitioner to join their comprehensive 
care team. This position is part-time, benefit
eligible, afternoons (M-F). This person will 
split their time between care coordination and
providing direct patient care for children with 
chronic conditions, such as ADHD, asthma, autism,
and behavior health issues. Duties will include 
care coordination, preventative health care
counseling, development of care plans, patient 
care and serving as a patient/family resource
and advocate. Prior experience working with 
chronically ill children is a must.

Park Nicollet Clinic and Methodist Hospital, is
recognized nationally for clinical quality, 
patient safety, disease management, and leadership.
