Axis Palliative Healthcare, LLC was created at the end of 2006 by 
LifePath Hospice and
Palliative Care, Inc. to meet the multidimensional needs of 
hospitalized patients with advanced
or chronic illnesses who are not yet appropriate for hospice care. 
Palliative care is similar to
hospice in that it aims to provide multidisciplinary care 
concentrating on maximizing quality of
life and relieving suffering. It is different from hospice in that 
palliative care can be appropriate
regardless of the prognosis or the type of treatment the patient is 
receiving. Axis Palliative
Healthcare helps extend and fulfill the mission of LifePath Hospice 
by caring for patients facing
serious life-limiting illnesses. As we continue our growth, we are seeking a:

Palliative Care Program Manager
Tampa, FL

S/he provides leadership and direction assuring high quality care for 
patients and families, and
the growth and development of the hospital and/or non-hospital based 
palliative care program.
S/he serves as mentor, educator and liaison between the program and 
community partners and
physicians, as well as with other health care professionals engaged 
in palliative care services.
Utilize clinical expertise and participate in diagnostic and 
management decision making while
promoting maximization of quality of life for the company's patients 
and caregivers.

Duties:
* Work with Medical Director in developing and implementing approved 
strategic plan for growth
and expansion of Axis Palliative Healthcare.
* Develop and enhance collegial relationships with community partners 
and potential partners of
Axis Palliative Healthcare.
* Introduce the program to assisted living facilities, nursing homes, 
insurance companies,
hospitals, and other potential locations that would benefit from the 
availability of a palliative care
program.
* Assist in developing and managing budget for Axis Palliative Healthcare.
* Monitor the data collected from the different care delivery sites 
and assist in maintaining or
changing practice patterns to ensure the highest quality of 
palliative care is available.
* Assist in the development of the Quality Assessment and Performance 
Improvement Plan (QAPI)
as well as implementing strategies to improve the indicators 
identified in the plan.
* Assist in developing and maintaining a medical record process that 
allows Axis to maintain its
own patient records within the organization.
* Represent Axis Palliative Healthcare on appropriate HPC, Inc. committees.
* Represent the Company at appropriate local, state and national forums.
* Assist in developing clinical practice standards for physicians and 
nurses in the practice of
palliative care.
* Assist in developing unique roles for physician extenders (ARNP, 
CNS, PA, etc.) in order to
expand palliative medical services in a cost-efficient manner.
* Work with Medical Director in developing and presenting reports 
regarding quality of care and
other outcomes as appropriate.
* Coordinate the work of medical staff and chair meetings as appropriate.
* Assist in maintaining compliance with all regulatory and 
accreditation standards pertaining to
delivery of palliative care in all settings.
* Assist in interviewing and selecting clinical and administrative 
staff for Axis.
* Represent Axis in contract negotiations with insurance companies 
and inpatient/outpatient facilities.
* Develop a plan of care for each palliative care patient/family with 
coordination of the patient's
attending physician, other consulting physicians and all medical 
staff of Axis Palliative Healthcare.
* Provide clinical expertise and participate in interdisciplinary 
team decisions regarding prognosis
and medical management of patient/family needs, assuring that the 
highest quality of medical care
is provided and that continuity and comprehensiveness of care is maintained.
* Cooperate with the patient's attending physician and other 
consulting physicians to coordinate
an appropriate palliative plan of care.
* Provide or coordinate direct patient palliative care through 
hospital visits, nursing home/assisted
living facility visits, or clinic visits as appropriate.
* Work cooperatively with and report to the Axis Medical Director to 
ensure quality care delivery to
all Axis Palliative Healthcare patients.
* Assure response time to consultation requests occur in a clinically 
appropriate manner.
* Provide coverage in the event of absence of other medical coverage.
* Provide comprehensive and efficient direct primary or consultative 
patient care under protocol
with Axis physician staff.

Qualifications:
* Masters degree in nursing (MSN)
* Full-time position; must be able to work weekends and be a part of 
Axis's call schedule rotation.
* ARNP certification recognized by CMS for Medicare billing
* Licensure by state of Florida to work as ARNP
* Knowledge and experience in palliative medical practice and symptom 
management
* Advanced clinical skills which allow thorough patient assessments 
and management for any
patient with advanced chronic disease as well as for those near the 
end of life
* Ability to work under protocol with multiple physicians in multiple settings
* Basic computer skills including use of email and word processing 
(Microsoft Outlook and
Word or equivalent programs)
* Strong interpersonal skills, communication, collaboration and 
conflict resolution skills / Ability
to perform presentations in front of large to small audiences.
* Ability to communicate with community providers regarding patient 
treatment options
* Ability to organize resources appropriately; develop budget and 
monitor/assess program
operations
* Ability to work independently with other disciplines within the 
organization, exercising
discretion and independent judgment
* Valid driver's license and automobile insurance
* Ability to travel to off-site locations
* Ability to manage multiple priorities simultaneously and 
effectively handle the emotional
stress of the workload
* Must be able to physically access any home in any area of Company 
service area, i.e.,
navigate stairs, and narrow spaces, tolerate heat and lack of air conditioning
