JBFCS, one of the nation's premier mental health 
and social service agencies, offers a diverse
network of approximately 175 community-based 
programs, residential facilities and day-
treatment centers. These programs serve more than 
65,000 people of all religious, ethnic and
economic backgrounds each year at sites 
throughout New York City and Westchester.

Nurse Practitioner – Westchester

The nurse practitioner will provide direct client 
care for adults with disabilities and coordinate
medical care and referrals for these clients.

Responsibilities include but not limited to:

*Primary care of residents
*Admission and discharge physical exams
*Arrange and coordinate subspecialty care including eye and dental exams
*Diagnose and manage acute and chronic illnesses of clients
*Act as a liaison with psychiatrist and provide appropriate medical follow-up
*Maintain appropriate client and staff records in 
accordance with JBFCS policies
*Partner with nursing staff on client care issues
*Work as a member of an inter-disciplinary team 
and attend all required meetings
*Train staff in regards to health related issues and job functions
*Ensure quality care is provided to clients with 
disabilities at responsible sites

Requirements: Nurse practitioner licensed to 
practice in the State of New York, 3-5 year clinical
experience providing direct patient care.
