Assignment duration: 8/15/11-9/16/11 *
Indian Health Facility, Outpatient Clinic
Monday-Friday 8am-6pm, Thursday 8am-9pm
Provider will be responsible for diagnostic, preventative, and 
therapeutic health services
Duties include: complete medical screening; physical examination and 
diagnostic testing
Certification required: BLS, DEA
Requirements include: computer skills, updated shot record, background 
check, and
2 reference letters
Generous housing and travel allowance provided
Any active state license required
