Serving Washington County with offices in Machias & Calais*

Community Health and Counseling Services has been awarded a federal 
grant as
lead agency to provide integrated primary and behavioral health care in 
the form of
ongoing and preventative health care, patient education, wellness 
activities and care
management services to individuals with severe and persistent mental 
illness residing
in rural Washington County.

The position will provide comprehensive health assessments on clients 
with severe
mental illness and/or substance abuse upon referral from Nurse Care 
Managers or
Case Managers. When the assessment determines that the client is high 
risk due to
complex medical/behavioral issues, the provider makes a referral to the 
Wraparound
Medical Team. Must have a Maine license as either a Physician's 
Assistant or a Nurse
Practitioner and have 1 year of primary care experience.
