Nurse Practitioner, Clinical Quality Review

As the Core Measure Transitional Practitioner, 
you will serve as the AMI, CHF, Pneumonia &
SCIP Practitioner, working directly with the 
Medical Staff, Nursing Staff and Quality Department
to improve overall core measure compliance. This 
position carries responsibility for
transforming care and facilitating ongoing 
compliance with core measure standards through
chart review and direct communication with 
physicians and staff. Duties include developing
and working under protocols approved by the 
medical staff to improve core measure
compliance and patient outcome.

The successful applicant will be a dedicated 
Nurse Practitioner with 3+ years of acute care
experience, outstanding 
communication/interpersonal skills, individual accountability, and the
desire to be a team player. Master’s degree 
required; knowledge of core measure guidelines
preferred.
