Swedish Medical Group, a network of 17 primary-care clinics and 56+ 
specialty clinics within the
Swedish Health System, has a long history of providing excellent patient 
care throughout Greater
Seattle. We are amidst an exciting time of growth and opportunity at 
Swedish, including building
our new MOB & Hospital in Issaquah, the opening of two Ambulatory Care 
Centers and assuming
operation responsibility at Stevens Hospital in Edmonds, WA.

We are excited to announce the implementation of the Residential Care 
Team service line at
Swedish Medical Group. Residential Care Team providers primarily serve 
medically complex
patients in post-acute settings who are not able to easily access 
primary care clinics. They actively
manage patients in transition: ensuring coordinated care, enabling the 
best possible clinical
  outcomes, and supporting patients' safe return to primary care and home.

We are recruiting qualified Nurse Practitioners to work in a 
collaborative model to provide
comprehensive medical management for patients in skilled nursing 
facilities for sub-acute or long
term care. As the Nurse Practitioner, you will be primarily responsible 
for the management and
treatment of patients with acute and chronic health conditions in an 
assigned skilled nursing and
long-term care facility. Along with a collaborating attending physician 
and the facility's
interdisciplinary team, you will direct plans of care, prepare and 
maintain accurate patient records,
prescribe medications, and coordinate care as they transition between 
care settings.

A Master's Degree in Nursing, a current WA State Medical RN and AP 
License and Federal DEA
Registration are also required. Qualified candidates will have a minimum 
of two years of
experience working in hospice, geriatrics, and/or intensive care.
