American Telecare, Inc (ATI), headquartered in Eden Prairie, 
Minnesota, combines technology
and expertise to create practical, clinically-based solutions to help 
improve patient outcomes,
resulting in lower costs. We are recruiting nurse practitioners to 
work in our Advanced Care
Management program in Minneapolis, Minnesota.

Description
The Nurse Practitioner (NP) role is responsible for the management of 
a panel of patients
enrolled in ATI's Advanced Care Management program to achieve the 
objectives of reducing
patient hospitalizations , ER visits, and improving their quality of 
life, activation and coordinating
their care. The NP establishes an individualized plan of care for 
each patient, monitors patient
biometric data and intervenes based on advanced practice role, 
clinical judgment and program
policy. The clinician will perform regular assessments of enrollees 
using a combination of
telehealth monitoring and real time telehealth sessions. The NP's 
collaboration with internal
and external care team members to identify and support appropriate 
care coordination is
important to the patient's success and the improvement of their health.

Requirements
The ideal candidate will be self-directed, possess highly developed 
critical thinking, problem
solving and time management skills. Also important to the role is:
* expertise in motivational interviewing,
* willingness to work in an innovative and changing clinical practice 
supported by telehealth
technology,
* experience with community based health care programs and resources,
* embracing the concept of practicing in a non-traditional patient 
care paradigm with evolving
clinical programs,
* desiring to work effectively with a team and to participate in 
overall program development,
* demonstration of competence with technology including MS Office 
products and the Internet;
willing to learn and embrace telehealth technology as a tool for 
clinical care delivery,
* and to be creative, energetic and flexible.

Current MN RN license, NP certification and DEA registration is 
required. Five years of
experience as a Nurse Practitioner with broad clinical experience in 
multiple settings and
approaches is preferred.
