Rural Health Clinic – Family Nurse Practitioner 
or Physician Assistant – Black River Falls,
Wisconsin

We are seeking a Family Nurse Practitioner or 
Physician Assistant interested in working in a
Rural Health Clinic affiliated with a well 
respected hospital. The Provider would work in an
autonomous setting with an LPN for support. The 
physician comes once per week to review
charts and is available via back-up call. The 
clinic hours are Mon-Fri 8:30am-5:00pm with an
estimated volume of 12-15 patients per day. The 
Provider would also be responsible for one
weekend of call per month in the ED. Compensation 
package including incentives of $80,000-
$85,000 with paid ER call an additional $6,000 
per year. Full Benefits package including
Medical, Dental, Retirement, 4 weeks paid 
vacation, $2,500 CME Allowance and 5 days of CME.
