Park Nicollet Clinic is seeking a FT Family Nurse Practitioner to 
join its new clinic opening in
Lakeville, MN Summer 2011. This is a fantastic opportunity to join a 
dynamic care team. You
will split their time between providing direct patient care and care 
coordination for patients with
chronic conditions, such as ADHD, asthma, diabetes, hypertension and 
autism. Duties will
include care coordination, preventative health care counseling, 
development of care plans,
patient care and serving as a patient/family resource and advocate.

Park Nicollet Health Services, which includes Park Nicollet Clinic 
and Methodist Hospital, has
been repeatedly recognized nationally for quality improvement and 
care activities. Park Nicollet
clinic is one of the largest multispecialty clinics in the United 
States, with over 600 clinicians
providing care in 45 medical/surgical specialties and subspecialties.
