Palliative Care
Advance Practice Nurse

Park Nicollet Health Services is seeking an 
outstanding Palliative Care Advance Practice Nurse
to join a thriving inpatient and outpatient 
Palliative Care program. Join a dynamic and dedicated
interdisciplinary team of 3 Palliative Care 
physicians, a social worker, a chaplain and a nurse
coordinator. The Advance Practice Nurse will 
function as part of the Palliative Care team to
assess the needs of palliative care patients and 
families and assist in the management of
complicated symptoms and goals of care 
discussions. NP, CNS or PA considered. Palliative
Care or Hospice education or experience preferred. Excellent teamwork, symptom
management and communication skills are a must.
