*Adult Nurse Practitioner Needed in North Chicago Pain & Rehabilitation 
Clinic*

Full-time and Part-time positions available
Monday-Friday 8am-5pm

This position offers an opportunity to an individual who will work 
collaboratively with
a physician and other multidisciplinary healthcare members to provide 
quality
management of pain and rehabilitation needs. You will be responsible for 
providing
direct patient care, consultation and & education as outlined within the 
collaborative
agreement. You will work independently to obtain medical histories, perform
appropriate physical assessments and examinations, have prescriptive 
authority
per agreement, order diagnostic tests, and collaboratively formulate a 
plan of care
for rehabilitation and management of pain. Excellent communication and
interpersonal skills will facilitate delivery of exceptional health care 
to our patients.
