Nurse Practitioner
Diabetes Educator/Program Coordinator
Patient Care Services
F/T Days

*The Nurse Practitioner Diabetes Educator/Program Coordinator is 
responsible for
program development, including performance improvement, implementation of
services and patient education for both inpatients and outpatients and 
their
families. Candidate will collaborate with the healthcare team related to 
diabetes
care management; develop and implement Clinical Policies and Procedures and
participates in staff development; and facilitate the Diabetes Advisory 
Committee.


*Responsibilities include but not limited to: *
*Performing clinical assessments, planning, implementing the Center for 
Diabetes
program goals, objectives, policies and procedures. In conjunction with 
the OB GYN
physicians, will expand the current Gestational Diabetes Program to include
medication management.
*Formulate, provide and evaluate diabetes education for patients 
utilizing standards
of the American Diabetes Association and the American Association of 
Diabetes
Educators.
*Assess the educational needs of individuals enrolled in the outpatient 
Diabetes
Education Program.
*Maintain documentation records of educational services provided to 
outpatients
enrolled in Diabetes Education Program.
*Plan with patient, mutually agreed upon goals interventions.
*Evaluate the patient's plan of care through follow-up.
*Collaborate with managers and physicians to assess and evaluate education
provided to patients with diabetes.
*Ensure completeness of all direct and indirect patient services 
including pharmacy
prescription, ancillary health referrals, and overall program 
effectiveness. Consults
with referring physicians regarding their patients and assists patients 
with needed
referrals for primary care and specialists' services.
*May provide direct patient care as a nurse practitioner and diabetes 
educator and
serve as a resource for nursing staff.
*Collaborate with Care Managers to evaluate diabetes competence of staff.
*Chair the interdisciplinary Diabetes Advisory Committee.
*Participate in health fairs and outreach to the community.
*Prepare and manage the operating budget for the Diabetes Center.
*Collaborate with the Finance Department to analyze reimbursement data 
to develop
increase revenues from Outpatient Services.

*Qualifications:
**Currently licensed as a Nurse Practitioner and Registered Nurse in New 
York State.
*Graduate from an accredited Masters of Nursing Program.
*Certified or certification eligible as an Advanced Nurse Practitioner 
by the American
Nurses Credentialing Council.
*Currently certified or eligible as a Diabetes Educator (C.D.E.).
*At least 3 years clinical experience with diabetes patients.
*Gestational diabetes management experience preferred.
