House Calls Medical Solutions, a new medical practice is a home and 
community
based primary care practice- providing care and services in the seniors 
home. The
NP will be a leader of a patient focused care plan team, working with a 
medical
assistant and using your clinical knowledge and excellent assessment and
diagnostic skills in working with other collaborative partners in 
meeting the needs of
a senior home based population.
