*Seeking Advanced Practice Nurse--Cleveland, Ohio *

Our center for older adults, a leader in faith-based care and services 
for older adults
in Northeast Ohio, is seeking an Advanced Practice Nurse to positively 
impact
quality outcomes for patients served in our skilled nursing facility and 
other service
areas on our campus including daily rounds, and medically necessary patient
evaluations. An initial focus area for this position will be to champion 
the
implementation of a heart failure disease management program in our 
short-term
rehabilitation/skilled nursing area. This program has already been 
developed by a
local geriatric cardiologist. We are seeking a part-time or full-time 
Nurse Practitioner
or Clinical Nurse Specialist with hands-on patient care experience and 
prescriptive
rights. Experience with the older adult population is preferred.

Qualifications: *
*Licensed in Ohio as an Advanced Practice Nurse
*Training or Practicing Experience in Adult or Geriatric Medicine

**Part-time 10am-2pm, Monday--Friday **
**Full-time 10am-6:30pm, Monday-Friday **
