Amazing opportunity in Southwest Arizona for an experienced NP to join Yuma
Regional Medical Center's Heart Failure Program. The HFNP - led visits 
are to
ensure appropriate patient transitions from the inpatient to outpatient 
setting, to
include verification that the patient has obtained medications, understand
medications, is taking medications and understands the diagnosis of heart
failure, including the signs and symptoms of worsening disease. Position 
has
an expanded role, assessing patients, analyzing data, working closely with
colleagues to ensure integration of services and continuity of care for 
heart failure
patients. The HFNP will play a pivotal role in the Heart Failure 
program, working
with care providers and administration on quality initiatives, improving 
patient
satisfaction and reducing readmission rates. Must have 3-5 years 
experience as a
Nurse Practitioner with cardiovascular patient care experience. Licensed 
in the
state of Arizona. Completion of the minimum required for State or National
Certification with certification in the NP's area of specialization 
(adult, family, etc) as
required by the Arizona Board of Nursing. We offer competitive pay and
