Current license required.
Green Bay, WI

MAJOR JOB FUNCTIONS: *
* Performs a face-to-face encounter for Medicare patients as assigned by 
the
Clinical Director or designee.
* Documents clinical findings of face-to-face encounter. Documentation 
should
include:
      1. Evidence of decline in clinical status.
      2. Evidence of non disease-specified base guidelines, i.e., PPS, 
Karnofsky,
dependence in ADL's.
      3. Evidence of co-morbid conditions.
* Communicates clinical findings to the physician who is considering 
recertifying
the patient.
* Completes attestation statement regarding date of face-to-face encounter.
* Performs medically reasonable and necessary patient care as needed during
face-to-face encounter.
* Submits accurate time and mileage records for all patient related 
activity to the
Clinical Director on a bi-weekly basis.

*REQUIREMENTS: *
* Licensed as a Nurse Practitioner in the state of practice.
* Must be able to maintain patient confidentiality at all times and be 
able to abide
by HIPAA rules and regulations
* Must be proficient with computers, and hand held technical devices 
such as
Smart Phones
