*Risk Adjustment Program Medical Evaluator **

**POSITION/TITLE: Medical Evaluator **
**DEPARTMENT: Medical Management **
**
**Position purpose: *
Medical Evaluator will complete detailed comprehensive health assessments
("assessments") of high-risk members. These assessments include a 
face-to-face
visit where the member resides (private home or long-term care facility) 
or clinic
setting. The assessments help to identify candidates for disease and case
management interventions and are also shared with the member’s primary care
provider to address any medical follow-up care.

*Responsibilities / Duties / Functions / Tasks: *
*Prepares and reviews comprehensive medical histories and obtains data 
through
interviews.
*Examines patient and documents all current medical conditions.
*Interprets test and exam results to develop a comprehensive report of 
diagnoses
for ATRIO case management and quality improvement programs.
*Documents follow-up care recommendations when appropriate.
*Effectively represents the health plan in a positive manner to patients 
and providers.
*Properly identify and code medical conditions to the highest degree of 
specificity
while complying with current ICD-9-CM documentation requirements.
*Reinforces to members the importance of staying current on covered 
preventive
services.

Qualifications: *
Family or Internal Medicine Doctor of Medicine/Doctor of Osteopathy or 
Family or
Adult Nurse Practitioner. Licensure and board certification is mandatory.
*
**Experience: *
*At least 5 years of experience in primary care setting preferred.
*Computer skills including navigating Windows, Internet Explorer, 
Microsoft Word,
and Outlook.
*Knowledge of managed care and Medicare criteria preferred

*Working Conditions & Physical Requirements: *
Assessments conducted in private homes, long-term care facilities or 
clinic setting.
Daily travel required. Reimbursement for mileage provided according to 
federal rate.
