*In Home Primary Care- a home visit primary care practice serving frail 
elders in
Snohomish County.*

Seeking a FT or PT Adult, Family or Geriatric NP with a passion for 
serving the elders
to join our experienced NP group doing home visits. We serve patients at 
AFHs,
Assisted living facilities and private homes with a special focus on 
Palliative care.
The role is independent yet our team of NPs and a consulting MD provide 
mentoring
and assistance as needed by the individual provider. Specifically 
seeking a provider
to serve the north Snohomish County areas of north Everett, Marysville 
and Arlington.
