*Nurse Practitioner

As part of our multi-specialty medical group practice, and reporting to 
the Chairman
of Ambulatory Care, the highly motivated and energetic NP we seek will 
join a robust
team of professionals made up of Ambulatory Care Physicians, Mental 
Health Nurse
Practitioners, Medical Residents, Social Workers, Registered Nurses, Case
Managers, Licensed Practical Nurses, Patient Care Technicians, Clerical 
Staff, and
a Center Administrator. All of which are deeply committed to our mission of
strengthening our healthcare system, and providing outstanding care to 
the 33,000
patients we care for annually.

Along with providing direct patient care, you will play an integral role 
in the
supervision of services, manage care coordination utilizing our fully 
implemented
eHR, and participate in weekly meetings during our exciting 
transformation period.
Additionally, you will provide clinical care using a whole team approach 
centered on
patient needs, and incorporate individual preferences in the delivery 
and approach
to care. The ability to utilize and incorporate electronic tools, 
including EMR,
e-prescribing, patient registries, and electronic data relating to 
quality/patient
outcomes within the practice setting is essential.

*To qualify, you must have:*
* NYS Nurse Practitioner/ Registered Nurse Certification in good standing
* Current and valid BLS certification
* Ability to become ACLS Certified within 90 days of employment
* Possess prior experience working in a physician type practice or 
hospital based
outpatient multi service practice
* Minimum of three (3) years experience as a licensed RN, preferably with
experience in outpatient primary care
* Demonstrated leadership skills
* Excellent oral/written communication skills
* Care coordination and care management experience preferred
* Experience with patient-centered care models preferred
