We are seeking a Family, Adult or Geriatric Nurse Practitioner for our 
Home Care
Model in the greater Conroe Texas area. Our client focuses on a 
Patient-Centered
model that lowers cost and maximizes efficiency for homebound patients with
chronic needs. As a Provider, you will enjoy the training, clinical & 
administrative
support, autonomy and flexibility while Providing quality and 
compassionate care in
the patients home. The Nurse Practitioner will take vitals, perform 
medical histories,
performing diagnostic services and assessing patients' homes for safety and
security. The Provider will also be assessing and managing chronic medical
conditions. This includes care for all stages of life: acute care, 
chronic care,
preventative services and end of life care; as well as coordinating all 
products and
services to achieve the highest possible outcomes at the lowest possible 
cost.

*Qualifications:*

: Monday-Friday, 8am-5pm, no call, no rounds
