Senior LIFE Ebensburg is a program of all inclusive care for the elderly 
providing premier
healthcare to eligible seniors. This home and community based program 
focuses on the
quality of life of the participants enabling them to receive the 
coordination of services to keep
them at home. We offer a competitive salary based on experience, no

*Responsibilities:*
* Applies advanced knowledge/clinical skills necessary to assess/manage 
the healthcare of
chronically ill geriatric members in the Senior LIFE program.
* Focuses on empowering members with the goal of maximizing quality of 
life.
* Assess, initiate and coordinate member care on a daily basis under the 
supervision of the
attending physician
* Act as a liaison with physicians, nurses  and other healthcare 
professionals, providing
education, communication and support for healthcare staff, members and 
families

*Qualifications:*
* Must be a PA licensed CRNP from an accredited university
* A minimum of one year experience  with experience in geriatric care 
preferred
