*Palliative Care Nurse Practitioner*

Located in *Richland, Washington, Hospice at Tri-Cities Chaplaincy* is 
seeking a dynamic *
Nurse Practitioner* to participate as a clinical leader with Hospice and 
Palliative Care
interdisciplinary teams to provide specialized out-patient consulting 
services in homes and
LTC facilities. Provide initial consultation for referring physicians in 
patient homes and LTC
facilities.

* Provide advance practice nursing services:
- Assessment -- comprehensive or focused as necessary for the 
patient/family needs.
- Initial and follow-up clinical care in patient homes and/or Adult care 
facilities.
- In collaboration with the interdisciplinary team, provide counseling 
and education to patients
and families to facilitate patient-focused and value-driven care. 
Facilitate coordination and
goals of care.
- Initiate and monitor symptom management interventions in active 
collaboration with patient's
attending physician.

* Foster professional relationships with physicians, nursing leadership 
and staff in all settings
of care.

* Develop and carry out patient management plans within the context of 
the interdisciplinary
team.

* Hospice support as needed. Approximately 25% of time.

Employment Type: Full Time, Exempt

Education: Advanced Registered Nurse Practitioner with prescriptive 
authority

Experience: ARNP experience in one or more of the following: Internal 
Medicine, Family
Practice, Oncology, Hospice or Palliative Care; 3 yrs. experience as a 
staff nurse

Travel: Road Warrior
